Plugin doesn't actually populate the "q" queryset

I've implemented the plugin on a super-simple search form. Everything works grand except....the "q" (queryset) doesn't ever get populated with the values, so when someone finally submits the search query, they always get nothing.

So the dropdown list of values works fine and the user clicks on an item. It appears on the page. But when they click the 'Submit' button, the URL is something like this:

"as_values_xx" has something, but the "q" parameter has nothing.

My HTML is simple, just a form like this:

Any ideas?
